Auch im Jahr 2008 hatte das VIVIT wieder Grund zu feiern.
Das Vorarlberg Institute for Vascular Investigation and Treatment (VIVIT) am Akademischen
Lehrkrankenhaus Feldkirch hat sich in den vergangenen 10 Jahren auf internationaler Ebene als
ein führendes Zentrum der medizinisch-klinischen Forschung etabliert.
Am Freitag, den 26. September fand aus Anlass des 10-jährigen Bestehens des VIVIT Instituts
ein internationaler Forschungskongress im LKH Feldkirch statt. Praxisrelevante neue
wissenschaftliche Erkenntnisse aus den besonders wichtigen Fachbereichen Diabetes,
Übergewicht, Fettstoffwechselstörungen, und Herzerkrankungen wurden vorgestellt.
International herausragende Experten berichteten über entscheidende Entwicklungen in diesen
Fachbereichen; Forscher aus dem VIVIT Institut haben die Ergebnisse der eigenen
wissenschaftlichen Arbeit vorgestellt.
Der Kongress wurde von Landesstatthalter Mag. Markus Wallner eröffnet. In seiner Ansprache
würdigte er die zentrale Rolle des VIVIT für die medizinische Forschung Marke Vorarlberg.
Prof. Haffner aus den USA referierte über den Nutzen einer intensiven Blutzuckersenkung zur
Vermeidung von Herzkreislauferkrankungen. Über den Nutzen neuer Medikamente in der
Behandlung von Patienten mit hohem Blutzucker sprach Prof. Ludvik aus Wien. Doz. Säly aus
Feldkirch hat neue Ergebnisse einer Studie bei 750 Vorarlberger Patienten präsentiert.
Prof. Patsch aus Salzburg sprach über die Rolle von Entzündung in der Entstehung der
Insulinresistenz. Prim. Höfle aus Hohenems berichtete über die Rolle des Fettgewebes als
Produktionsort für Hormone. Aus Wien stellte Prof. Wascher konservative und chirurgische
Möglichkeiten zur Behandlung der Fettleibigkeit vor.
Prof. Kontush aus Paris stellte neue Fakten zur Bedeutung des guten Cholesterins (HDLCholestern) vor, anschließend berichtete Prof. März über Studienergebnisse zur Senkung des
schlechten Cholesterins. Aus St. Gallen referierte Prof. Riesen über die Bedeutung des
Cholesterins für die Entstehung eines Schlaganfalls, bevor Dr. Rein aus Feldkirch über die
Bedeutung einer eingeschränkten Nierenfunktion für die Entstehung von
Herzkreislauferkrankungen sprach.
Von der Universitätsklinik in Innsbruck berichtete Prof. Pachinger über aktuelle Kontroversen in
der Therapie von Herzerkrankungen. Prof. Amann aus Zürich präsentierte aktuelle diagnostische
und therapeutische Möglichkeiten von Herzkatheteruntersuchungen und warf dabei auch einen
Blick in die Zukunft. Aus Feldkirch präsentierte Prof. Drexel Erkenntnisse über die
Zusammenhänge von Stoffwechsel und Herz – ein noch sehr junges Wissensgebiet. Abschließend
referierte Doz. Festa über den Prozess der Entwicklung neuer Medikamente.
Altlandeshauptmann und VIVIT-Vorstandspräsident Dr. Martin Purtscher beendete den Kongress
mit einem Resumee der Erfolgsgeschichte des VIVIT im ersten Jahrzehnt. Er hob die engagierte
Rolle von Prof. Drexel und seinem Team hervor.
Die internationalen Referenten zollten dem VIVIT in hohem Maße Anerkennung. Prof. März aus
Heidelberg betonte „Diese Forschung in Vorarlberg wird international stark wahrgenommen“.
Prof. Schernthaner aus Wien hob die Bedeutung des VIVIT in der österreichischen
Forschungslandschaft hervor. „Es ist entscheidend, dass nicht nur die staatlichen Universitäten in
der Forschung aktiv sind, sondern auch Schwerpunktkrankenhäuser.“
Das Jahr 2008 war im VIVIT-Institut auch wieder geprägt von vielen Studien, sowohl
Pharmastudien als auch Eigenstudien.
Es wurden im vergangenen Jahr etablierte Projekte fortgeführt und neue Projekte begonnen.
Derzeit liegt die Konzentration auf der Fortführung der „PAVK Studie“. Bisher haben wir hier
über 200 Patienten eingeschlossen, die an einer peripheren arteriellen Verschlusskrankheit leiden.
Ziel der Studie ist es, verschiedene Risikofaktoren für eine PAVK zu identifizieren und zu
quantifizieren. Untersucht wurden u.a. Labormarker wie Blutfette und Entzündungswerte, aber
auch „neuere“ Risikofaktoren wie die Körperfettverteilung und die Knochendichte, sowie
genetische Assoziationen. Neben physischen Risikofaktoren wurden zusätzlich psychische
Faktoren der Krankheitsentstehung mittels eines ausführlichen Fragebogens untersucht. Die
Studie wird in Kooperation mit der Universität Bern, Klinik für Angiologie unter der Leitung von
Prof. Dr. I. Baumgartner durchgeführt.
Über den Sommer konnte erfolgreich in altbewährter Form das 8-Jahres-Follow up der
Koronarstudie I mit über 500 Patienten durchgeführt werden.
In den Monaten August bis Ende Oktober lag unsere Konzentration auf der Fortführung der
Karrenstudie. Erneut haben 58 hochmotivierte Probanden über 8 Wochen teilgenommen. Jeweils
am Beginn und am Ende der Trainingsperiode wurde ein intensives Untersuchungswochenende
veranstaltet, an dem unzählige Parameter erhoben wurden. Diese Studie wurde in
Zusammenarbeit mit der Abteilung für Physikalische Medizin unter der Leitung von Prim.
Bochdansky durchgeführt.
Weiters läuft seit kurzem auch eine Studie mit Patienten, die aufgrund eines ACS (akutes koronar
Syndrom) an unserer CCU aufgenommen wurden. Auch hier sollen Risikofaktoren und
genetische Zusammenhänge möglichst genau erforscht werden. Bei dieser Studie sind
Kooperationen mit den Abteilungen für Innere Medizin in Bregenz und Hohenems beschlossen.
Im Planungsstadium befindet sich eine weitere Studie mit Herzinsuffizienz Patienten.
Wir haben im VIVIT Institut, parallel zu unseren eigenen Projekten, derzeit auch mehrere
Auftragsstudien in Arbeit. Diese Studien umfassen vor allem unsere Spezialgebiete,
Lipidstoffwechsel und Diabetes, aber auch kardiologische und angiologische Bereiche.
Im speziellen ist derzeit eine Studie, die Einstein Studie, mit einem direkten Faktor X Inhibitor,
Rivaroxaban, welcher in Zukunft aufgrund verschiedener Vorteile die herkömmliche
Antikoagulation mit Sintrom oder Marcoumar ersetzen soll, zu erwähnen.
Weitere Studien mit Lipidsenkern und Antidiabetika werden parallel betreut.
Das 2007 neu bezogene Molekularbiologische Labor des VIVIT konnte den Erfolg seiner
Tätigkeiten im Jahr 2008 fortsetzen. Auch 2008 gelang es neue Zusammenhänge zwischen
genetischen Risikofaktoren und kardiovaskulären Erkrankungen auf internationalen Kongressen
zu präsentieren und in hochrangigen wissenschaftlichen Journalen zu publizieren. Ebenso wurde
das 2007 begonnene Projekt über den Einfluss genetischer Faktoren auf den Dickdarmkrebs
höchst erfolgreich weitergeführt. Hierbei zeigte die genetische Untersuchung des Onkogens KRAS einen mutationsabhängigen Einfluss auf den Verlauf der Krebserkrankung. Für die
Präsentation dieser Ergebnisse erhielt Dr. Thomas Winder den Posteraward der österreichischen,
deutschen und schweizerischen Gesellschaft für Hämatologie und Onkologie. Das im Vorjahr
gänzlich neu aufgebaute Projekt über das Arbeiten mit Zellkulturen lieferte 2008 die nötigen
Expertise für die Beantragung einer Förderung beim österreichischen Fond für wissenschaftliche
Grundlagenforschung (FWF), welche Ende 2008 bewilligt wurde. Da diese in der Regel nur
hochrangigen Universitäts-Instituten zuteil wird, ist dies für das Molekularbiologische Labor des
VIVIT eine besondere Auszeichnung. Neben rein wissenschaftlichen Tätigkeiten erfolgte die
Etablierung eines breiten molekulargenetischen Diagnostikangebotes, welches unter anderem
hoch sensitive Untersuchungen bei Bluterkrankungen und bei genetisch bedingtem
Nichtansprechen von Medikamenten umfasst. Die in Kooperation mit dem Medizinischen
Zentrallabor Feldkirch angebotenen Analysen werden ab 2009 für die Diagnostik verfügbar sein.
Die wissenschaftliche Tätigkeit von VIVIT Nephrologie umfasste neben diversen Vorträgen an
nationalen und internationalen Kongressen vor allem die Publikation von Kooperationsarbeiten
mit der Abteilung für Nephrologie und Hypertensiologie, Med.Univ.Klinik Innsbruck und der
Abteilung für Genetische Epidemiologie, Med.Univ.Innsbruck (Zitt, Neyer; Details siehe Bericht
der Abteilung Nephrologie).
Größten Arbeitsaufwand erforderte die Datenerfassung und –Aufarbeitung unserer prospektiven
Beobachtungsstudie einer Kohorte von 250 inzidenten Dialysepatienten in Vorarlberg, die Ende
2007 nach einer mittleren Beobachtungsdauer von 40 Monaten erfolgte (INVOR-Study). Aus den
vorliegenden Daten dürften in den nächsten Jahren einige Publikationen erwachsen.
Mit Ende des Jahres 2008 geht die wissenschaftliche Leitung von VIVIT Nephrologie von
Prim.Dr.U.Neyer auf Prim.Univ.Doz.Dr.K. Lhotta über.
All die beschriebenen Erfolge wären jedoch ohne die finanzielle Unterstützung der Sponsoren
